What learning difficulties does * have?
What shows that your child needs special help and support?
Has your child been diagnosed with anything particular? —give details of the diagnosis, when it was made an who by.
What difficulties do you think your child might have during the school day, including learning and social aspects?
Think about this in relation to:
Has school discussed these needs with you?
A Yes or No answer or state those things that haven’t been discussed.
When? e.g. informally with a member of staff and/or at any meetings. Give the approximate date of the most recent meeting and how often they are held.
Do you know what arrangements the school has made for * ?
Yes or No
What are they? e.g.
Put the number of Support hours per week if known and how they are used.
Do you see your child’s IEP (Individual Education Plan) and are you involved in planning and reviewing it? If so, how often?
Why do you think it is necessary for the LEA to assess *’s special educational needs?
What worries do you have about *’s schooling at the moment or in the future?
Do you want to expand on any of the difficulties listed in question two?
Are there any examples of particular problems or successes that * has had at school?
